INTRODUCTION

Kigoma rural district is one of few districts observed to at risk district on infant and martenal mortality rate from 2007 to date. The rate is increasingly fast to the extent that according to findings, about 2753 children and pregnant women die after every year. the situation covers areas around lake Tanganyika comuunities who are concevertive to their cultural value and takes considerable time to refferal hospital for further services. I addition, these communities are reluctant to get birth to dispensary, take their children to clinic and practice family planing. The organization see a need to have intervation up on these community for their bettermmment and National at large.

 

EXECUTIVE SUMMARY

Women Empowerment and Rural Development Organization is looking for fund to implement our TUPAMBANE program with the objective of building awareness on Women health with regard to Reproductive health, providing rural women with right and relevant health knowledge and information, assisting rural women on how to effectively apply and practice health information, and train them on how to evaluate changes in infant and maternal mortality rate in their community. The program outcome is that, at the end of the program, rural women will be good representative to their community as well as training partners in the aspect of health knowledge and information, measurable increased in family planning among rural women who are conservative to their cultural practices, measurable increased number of rural people providing blood to serve vulnerable women (blood donation) at the end of the program, increased of number of rural women attendance at clinic and bear at health centers and dispensaries, measurable increased of rural women making practice of health information and knowledge. Additionally, by the end of the program, it is expected that reduced infant mortality and maternal death rate induced from accurate and effective utilization of health knowledge and information. The Women empowerment and Rural Development Organization (WERUDO) request funding  amounting to  Tsh. 45, 000, 000/= for program personnel, flip charts, mark pen, not book, pens, lime papers, chairs, laptop, printer and tables, wages to teaching partners, and conduct forum discussions on Radio to get hold program goal.

WERUDO will enable both pregnant and non pregnant rural women to acquire and improve their health knowledge, make application of correct and relevant health information and knowledge, practice effectively family planning, becoming a stone corner, confident, and committed health ambassadors in their community, becoming fore front in blood donation during the final program duration.

Through utilization of twenty three (23) personnel from various community angles who will be teaching partners and paramount in awareness building in Mahembe ward composing four villages, WERUDO will effectively arrive at pre-determined goal and objectives. Personnel will be equipped with full meals and accommodation while in the field program, using this program community member will be in the position voluntary to donate their blood to serve at risk women pregnant with inadequate blood and would have born as well as unite acquired health knowledge in other related community intervention.

TUPAMBANE program will cover four (04) villages, of which each village will generate ten (05) groups of women, each composing limited members of less than thirty (30) who will remain as representatives in their community about health related issues after the program. The program will last for five (05) months from August 2013 to December 2013 .

WERUDO in its operation, Governing board made up community leaders and WERUDO’s staff will operate to provide overall sanctioning of the program area. Periodic evaluation through focus group discussion and semi-structured interview will be employed to assess the value of the grogram under WERUDO on:-

(a)    The important significance improvement changes in health knowledge and information to rural women and community at whole during and after the program.

(b)   The significance of the program

(c)    Development of new insight on health knowledge and information

(d)   Application acquired health knowledge

STATEMENT OF THE PROBLEM

Women Empowerment and Rural Development Organization has over twenty (31) villages receiving services from the organization, of which all have been determined to be at risk and experience infant and maternal mortality in their development process because of variety reasons including, blood deficit to pregnant and non pregnant women, poor family planning practice, lack of adequate and sufficient health knowledge and information, and poor cultural practice.

Following research conducted by the organization in partnership with women human rights social and economic development, (WHRSED), Women promotion center, (WPC), and Local government Agencies on infant and maternal mortality using simple interview and household survey, 56.7% community members shows that, rural women and community as whole live under adequate health information, the situation that leads to persistence of un defined and recognized methods of birth inducing health problem. Where by 32.4% recognized blood deficit in safety blood bank at health centers and failure to meet balance diet as accelerations to infant and maternal death that rural people fail to donate blood where necessary to assist pregnant women with blood deficit and other related circumstance, and food they eat is not balanced. Additionally, 11% of respondents portrayed poor family planning as another reason for rural women poor health leading to death. District health officer, (DMO) at Maweni referral hospital recommended that, there is an influx of mother to child HIV/AIDS transmission because of little health information to rural women, where by Local government Authority in Kigoma provides statistics that about 2753 women and child die after every three years because of blood deficit, poor family planning and poor application of little health information they have, (Ministry of health)

The organization put enough appreciate to Government efforts to reduce the incidence in both Urban and Rural community. Though, despite government efforts to improve health and reduce infant and maternal mortality rate, it is transparently that, considerable number of new would have born children and pregnant women die because of blood deficit during birth, blood deficit at health centers, poor health knowledge, information, and application of it, and bad cultural practices including encouraging traditional birth system and food taboo. Economically, women are focal point of production in both family and national level, they contribute about 75% in the Gross National product and about 85% of women engage in production to make family persistence and stability. Additionally, children and new would have born child, are nationally and globally expected future leaders, if well not protected and equipped with adequate and sufficient health knowledge and information we are likely to lack their potentialities and decrement in Gross National Product, inducing underdevelopment. Also family separation, orphanage, and street children being another considerable outcome of if not infant mortality and maternal death addressed.

The research portrayed from various secondary and primary data that, community covered with correct, accurate, and relevant health knowledge and information stand significantly in family planning, production process, life skills, voluntary blood donation, application of health information and knowledge as well as sense of we feeling and togetherness which is the key principle in community development programs.

TUPAMBANE program is a new program in Kigoma Rural that will operate in Mahembe ward composing of four villages for the purpose of the program; the emphasize of TUPAMBANE program is on application of correct, sufficient and relevant health information and knowledge, and voluntary blood donation for the health of new would have born, children, and pregnant women.

TUPAMBANE program is automatically and directly under structural materials and disseminated by Women Empowerment and Rural Development Organization (WERUDO). The TUPAMBANE program will be attached with the catch word of TUPAMBANE DHIDI YA VIFO VYA WATOTO WADOGO NA MAMA WAJAWAZITO.

 

GOAL AND OBJECTIVES

The goal of TUPAMBANE program is to reduce child and maternal mortality among rural women to the extent where they can effectively utilize health knowledge and information to improve and top-up quality of their health status and living span that will help them in participation of development programs in their community.

Project Objectives;

(i)  Building awareness on women health with regards to reproductive health.

(ii)    To provide rural women with right and relevant health knowledge and information.

(iii)To assist rural women in learning and how to make application and practice of relevant health information.

(iv)   To teach rural women on how to evaluate changes in infant and maternal mortality rate.

 

Project outcome;

(a)    Measurable reduced number of infant and maternal mortality rate in rural area is expected to be achieved to high extent.

(b)   Rural women will be good ambassadors to their community as well as teaching partners in the field of health knowledge and information.

(c)    A measurable and considerable number of rural women will be interested with program to the corner where will likely enjoy to get involved.

(d)   Application and practice of health knowledge and information timely and accurately by rural women will happen to get increased.

(e)    Increased women attendance to health centers and clinics inducing to get birth at dispensaries and health centers. Additionally, bad traditional practices and food taboo that negatively influence women health will no longer simply be not in practice at large.

(f)    Increased safe blood reserves in health blood bank as an output of voluntary community blood donation.

(g)   Measurable women will be subject to family planning because of relevant usage of health information and knowledge.

(h)   Reduced mother to child HIV/AIDS transmission.

 

PROJECT STRATEGIES/ACTIVITIES

1)      Formulation of women representative groups in every hamlet in the targeted villages. The activity is to have accessible and specific number of women ambassadors for effective knowledge transfer to the target group.

2)      Creation of center in every targeted village that will become a focal point for necessary discussion, events and teaching partners.

3)      Creation of center that will become a stone corner for training program to 15 teaching partners that will be sent execute TUPAMBANE PROGRAM in targeted villages.

4)      Recruitment of teaching partners and practically supervised for accomplishment of the program.

Formation of 30 tishetes with a catch word of TUPAMBANE DHIDI YA VIFO VYA WATOTO WADOGO NA MAMA WAJAWAZITO, to adress program theme.

5)      Implementation plan will be developed by staff to guarantee systematic collection and analysis of information about operation of the program, and provides the bases of sharing of information with similar projects.

ADMMINISTRATION STAFF.

The program will employ twenty three full times (23) and one (01) part time staff to facilitate the program to arrive at grogram goal and objectives. Staff to be developed is classified here under;

Ø  Project Director, (full time). This will be responsible for hiring project staff and teaching partners for the program accomplishment, overseeing project development and operation, establishing and maintaining link with women ambassadors from targeted villages and local agencies as well as budget.

Ø  Program Coordinator, (full time). This will be responsible for establishing centers, develop working relations with formal and informal community leaders, establishing link with community development organizations, health organizations, and health centers. Additionally, will be also in charge in recruiting teaching partners as well as women representative groups in hamlets with regards to local government.

Ø  Volunteer coordinators, (full time). These will work hand in hand with teaching partners in the targeted community. They will be responsible in corroboration with program coordinator recruiting teaching partners to facilitate the program. Establishing and maintaining linkage District Medical Officer (DMO), and private organized experts. Develop and offer training program to teaching partners, scheduling TUPAMBANE teaching partners in the field areas. The volunteer coordinator will have background on health knowledge doctors from Maweni referral.

Ø  Program evaluator, (part time). This will exactly measure the changes emanating from the program. She/he will also be responsible for collection of data regarding to persistence health status, health knowledge, and conducting periodic assessment in the level of knowledge comprehension and application of provided knowledge and information.

Ø  Teaching partners, (full time). These will be a stone corner for program sustainability, will be responsible for offering training via focus group discussion and walking around households in the target community. Additionally, training will cover both aspects, workshops in the identified canters and in the religious sessions, house to house, (walking around households), and focus group discussions with regards that, program goal and objectives met. Teaching partners will be an integral and community bed fellow to targeted community for direct importation of intended knowledge and information.

GOVERNING BOARD, this will dully made up both community leaders (local government Authority and WERUDOs staff. The board will observe sanctioning of the operation of centers, including teaching partners training center and those in hamlets for groups, and providing feedback to the project director on center policy and operation.
